Had same issue yesterday. I simply downloaded the bz stable version from github and haven't had a problem. https://github.com/SubnauticaModding/QModManager/releases/download/v4.3/QModManager_BZ.STABLE.zip
I had the EXACT same problem. You will need the version of Q mod manager directly from the Modding News section of their discord, not the nexus page. (Link: https://discord.gg/UpWuWwq ) If there is a mod you want to use that requires SML Helper you will probably want to use that version from the discord too.
