Autohotkey is generally the way to go for poe macros. something like this https://github.com/JoelStanford/PoEAutoFlask might be useful for you. Fairly simple scripts can also be made to activate the flask every 1/4 second when a button or key is held down (simple loop script). The poe playerbase has a lengthy history with ahk, so anything that you need has probably been at least touched on.

I use this one great script and explains how it's done so you can adjust it to your preferences, I personally edited mine with delay between 10-30 ms instead of 57-114
