It doesn't. ACE medical has had a bug since the medical update where any vest that uses a passThrough value of less than 0.2 in the hitpoints config causes units to take almost no damage. This affects certain vanilla vests as well.

Can somebody explain how does one get regenerative health while playing as a faction/single player with 3D-editor or missions in Arma 3. I also have ACE installed but can't seem to get it to work
Lol I don't think you're going to find that in Arma. ACE medical makes things more realistic, not less. Here's a brief guide on the basic and advanced medical system, it's a bit outdated at this point but still gives you a good overview of how it works: https://github.com/acemod/ACE3/blob/v3.12.6/docs/wiki/feature/medical-system.md

3CB Armour makes AI bullet sponges
ACE medical issue. Ever since they did their big medical system rewrite, it doesn't like vests that have a hitpoint passThrough value of 0.1 or lower. Vanilla content is affected as well, it's just not as noticeable because only a couple of vests use that value.
