How can I specify audio delay (e.g. in milliseconds)?
You can try this fork as well: https://github.com/987123879113/mame/wiki It's not officially supported, but the MAME dev who has been doing a ton of the 573 emulation work did this on the side.

Is there an emulator that runs Bemani games? (DDR/Beatmania/Pop'n Music)
SIO1 isn't properly emulated which stops beatmania IIDX 1st from booting (the DVD player error) and stops various Sys573 games from being playable: Martial Beat's I/O uses SIO1, Mambo a Go Go's rental system device uses SIO1, GFDM's card readers and multisession unit use SIO1. The system connection between beatmania IIDX and DDR also uses SIO1. In my branch I repurposed the i8251's code as a separate SIO1 device in my branch but in the process I think I broke P1's memory card when trying to use the branch as a PSX emulator directly. https://github.com/987123879113/mame/blob/bemani/src/devices/cpu/psx/sio.cpp

Is there a way for MAME to fill out the screen, when the monitor is in portrait mode?
As u/546897126 pointed out this cab isn't for MAME. I honestly posted this to put eyes on the cab and to put interest on Bemani MAME fork. Had to keep it relevant to the r/MAME sub

OS - Windows autobooting Big Box(Launchbox) which has Stepmania, running a commissioned vertical theme and a few arcade games that make full use of the vertical screen, while being able to center player 1 with this patcher. As well as a fork of MAME and a fork of PSCS2 playing their respective arcade games without audio/video sync issues.

Getting into iidx
8 note lanes can be hard to read for anyone just starting out on IIDX. I would also suggest giving BMS, osu!mania 7k, or even DJMAX and EZ2ON 6k/8k a try at home to start the reading brain training.

any rhythm games intro? similar to o2jam, or IIDX but on mobile or PC?
I also second Beatoraja and Lunatic Rave 2. Beatoraja is more modern, but can be fucky to set up if you don't know exactly how Windows locale shit works. LR2 is old as fuck and pretty jank, but it's been the standard for over a decade for a reason! You need to get your own songs for them though, they don't come with any defaults. However, the scene for them is MASSIVE so you will literally never run out of songs for them once you do find them.

You would have to install multiple programs. First, install LR2(http://www.dream-pro.info/\~lavalse/LR2IR/search.cgi?mode=download) or Beatoraja(https://github.com/wcko87/beatoraja-english-guide/wiki). These programs are use d to actually play BMS.
